Student of Concern Referral Form

The Students of Concern (SOC) program is operated through Student Affairs. Our mission is to serve as a campus resource team dedicated to assisting students who may be in distress or who may be experiencing challenging or difficult life circumstances. SOC works to coordinate and/or facilitate consultation and intervention when students exhibit aggressive, disruptive, or other concerning behaviors. The primary focus of the team is to take a proactive approach in identifying students who are struggling and provide early intervention, resources, and referrals both on and off campus.

Anyone who is aware of such a student is encouraged to alert the SOC through the following form. Please note that although we strive for confidentiality to the extent reasonably possible consistent with our legal and ethical obligations, we cannot guarantee it in all circumstances.

If you have questions, please contact the Student Affairs Office:

Phone: 401 454-6600
